Persistent Pain Management

Mobile physiotherapy to help improve quality of life with long-term pain.

Living with persistent or chronic pain can feel exhausting — physically and emotionally. At Jasmine Health Care, we provide informed, evidence-based physiotherapy to help you understand and manage pain in a practical, supportive, and effective manner.

We don’t take a “push through it” approach. Instead, we work with your body, your goals, and at your pace. Whether your pain is from arthritis, past injuries, surgery, or an unclear origin, we’re here to help you move more freely and with greater confidence. Whether you are a young person or an older adult, your pain story is unique and needs an individual approach.

A Supportive Approach to Persistent Pain

Our treatment is grounded in modern pain science, which tells us that persistent pain isn’t always a sign of damage but may result from how the nervous system responds over time. With the right approach and understanding of the pain, we can find ways to begin to support the body to move more comfortably again.

We will work with you to:

  • Understand your unique pain story and identify triggers
  • Rebuild confidence in movement through gentle, tailored activity
  • Explore strategies for pacing, relaxation, and flare-up management
  • Support day-to-day activities including walking, stairs and household tasks

We aim to reduce pain, the fear around it and help you feel more in control of your body.

Common Conditions We Support

  • Persistent back or neck pain
  • Joint and arthritis-related pain
  • Pain following surgery or injury
  • Fibromyalgia or generalised body aches
  • Pain that hasn’t responded to other treatments

What to Expect

Your first session will involve an assessment of your pain history, daily challenges, and what you’d like to improve. From there, you will have a step-by-step plan that may include targeted movement strategies, gentle strengthening exercises and practical advice on posture, pacing, and everyday activities. We also incorporate education around how pain works, to reduce fear and help you feel more in control.

It’s not always possible to eliminate pain overnight but we can help to improve pain and function over time — safely, gradually, and providing ongoing support.

Accessing Care

Persistent pain physiotherapy can be funded privately or through a number of options:

  • A GP referral for a Chronic Disease Management Plan
  • Support at Home (formerly Home Care Package)
  • Department of Veterans Affairs (DVA)
  • NDIS (if eligible)
  • Private health insurance

We’re happy to help you understand your funding options or work with your provider to get started.

Frequently Asked Questions

Yes. Physiotherapy can use gentle movement, education, and pacing strategies to help reduce sensitivity and improve function, even when pain has been present for years.

Persistent pain responds best to a different, more gradual approach than short-term pain. We will work with you at your current comfort level, focusing on you, not just giving you exercises to do. It’s not about doing more — it’s about doing what helps.

No, except to use Medicare, a GP referral is required under the Chronic Disease Management Plan.

We focus on safe, graded movement. Pain doesn’t always mean harm — we’ll work with you to move better, to know when to rest and when to persist gently.

Everyone is different. Some people notice changes within a few sessions, while others take longer. Our goal is steady, sustainable progress — physically and emotionally.
